Job Description

1.Conduct the daily nursing functions in accordance with current rules, regulations, and guidelines that govern home health care and in compliance with agency policies and procedures, professional and community standards.

2.Scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration and monitoring, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the RN Case Manager.

  1. Initial evaluation and ongoing comprehensive assessment of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at the appropriate time points.
  2. Initiate the plan of care and revise as needed.
  3. Provide services and/or treatments requiring substantial and specialized nursing skill.
  4. Counsel the client and family in meeting nursing and related needs.
  5. Initiate appropriate preventive and rehabilitative nursing procedures.
  6. Prepare clinical and progress notes and other clinical documentation per agency guidelines.
  7. Coordination of services and referral to other services as needed.
  8. Complete medication reconciliation and medication instruction with client participation.
  9. Notify physician of any variance in medications in home versus those ordered.
  10. Inform physician and other personnel of significant changes in the client’s condition and needs in a timely matter consistent with client needs.
  11. Evaluate outcomes of care.
  12. Assign home health aide to a specific client.
  13. Participate in in-service programs. Comply with the State Nurse Practice Act regarding continuing education and licensure renewal.
  14. Plan for client discharge from service.
  15. Provide skilled nursing visits for clients as directed by the RN Case Manager.
  16. Attend meetings as required.
  17. Communicate information on a timely basis, consistent with agency policy and urgency of the situation.  Communicate verbally, as needed and through complete and accurate documentation in the clinical record.
  18. Assist in plan of care.
  19. Assist client in learning appropriate self-care techniques
  20. Complete clinical documentation and reporting per agency guidelines.
  21. Participate in agency surveys (inspections) as requested by management.
  22. Perform job duties in accordance with agency policies and procedures as well as professional and community standards. Assure compliance with applicable state, federal, CHAP, and Joint Commission standards.
  23. Consistently demonstrate competency in technical nursing skills according to personal and legal scope of practice.
  24. Respect the confidentiality of information in client and employee records. Treat information in accordance with agency policy and HIPAA guidelines.
  25. Follow established agency guidelines for protection of confidentiality of all written and computer files. Protect personal password and secure written confidential documents in a manner that prevents unauthorized access or release.
  26. Participate in coordination of home care services.
  27. Maintain confidentiality in all aspects of the job.
  28. Participate in the development, implementation, and evaluation of the agency’s Quality Improvement Program and pertinent activities.
  29. Perform other duties and responsibilities as assigned.

Job Requirements

  • Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. 
  • Must be able to deal tactfully with staff, clients, vendors, government agencies/ personnel, and the general public. 
  • Must be able to work with ill, disabled, elderly, emotionally upset, and potentially hostile people.
  • Knowledge and understanding of the rules, regulations and laws applicable to home health agencies, including Medicare conditions of participation.
  • Must be able to speak and write English in a manner that is sufficient for effective communication.
  • Excellent oral and written communication skills.
  • Must possess leadership ability and willingness to work harmoniously with and supervise and manage personnel.
  • Must possess skills in leadership and problem solving.
  • Must possess creativity, integrity, and initiative. 
  • Must be able to provide critical thinking skills to address agency issues.
  • Must receive criminal clearance and other required screening prior to employment with the agency.
  • Must obtain required health screening and TB clearance (see also physical requirements below).
  • First Aid training.
  • Red Cross certified CPR training or equivalent.
  • Valid and current drivers’ license with appropriate endorsements.
  • Must be able to travel locally, including adequate automobile insured in accordance with applicable state law.

Education and Experience

Graduate of accredited school of professional nursing.

Minimum of one (1) year of experience in an acute care setting or equivalent experience.*

* Exceptions to the minimum experience requirement will be considered in extraordinary circumstances and must be approved in writing by the members of the Governing Body.


Must possess a current and active license to practice as an RN in the state of practice.
