Job Description

POSITION SUMMARY:  Provides personal care services under the direction of the Registered Nurse or Therapist.  The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services foe clients as per the plan of care.

FUNCTIONS/ACCOUNTABILITY:

 

  1. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist

    1. Range of motion

    2. Assistance in ambulation or exercises

  1. Performs personal care activities, including but not limited to:

    1. Bathing

    2. Shampooing

    3. Skin care/nail care

    4. Oral hygiene, shaving

    5. Dressing

    6. Shaving

  2. Performs household services essential to health care at home, including but not limited to :

    1. Meal preparation/feeding

    2. Laundry

    3. Light housekeeping

  1. Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse or therapist.  Follows reporting guidelines in the Care Plan.

  1. Documents care provided on the correct form and according to agency procedure.

  1. Promotes personal safety and a safe environment for clients by observing infection control practice, following agency guidelines and reporting unsafe situations to the Registered Nurse/Therapist or Administration.

 

  1. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.

 

  1. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences as requested.

 

  1. Attends in-service programs to meet compliance requirements.  Identifies learning needs to agency supervisor.

 

  1. Maintains confidentiality in all aspects of the job.

 

  1. Performs other related duties and responsibilities as deemed necessary.

Job Requirements

Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be able to work with ill, disabled, elderly, emotionally upset, and potentially hostile people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.
 

  • Successful completion of a formal 75 hour certification training program. 
  • Be at least eighteen (18) years of age 
  • Minimum of six (6) months work experience in a supervised setting, preferably health care facility 
  • Licensed driver with automobile insured in accordance with state laws and agency requirements and in good repair. Good driving record. 
  • Current CPR, current certification
